Your Rights
As our patient, you have the right to access your medical records, request amendments, restrict certain disclosures, and receive an accounting of disclosures. To exercise these rights, please contact our Privacy Officer. You also have the right to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
